ATHLETIC ASSOCIATION UNIFORM DEPOSIT
Volleyball/Basketball
(Applicable to 5th-8th graders only)
UNIFORM DEPOSITS FOR 5-8th GRADE VOLLEYBALL and BASKETBALL may be made at this time. If your 5th-8th grader(s) will be playing either or both of these sports, a one-time $40 uniform deposit for each athlete must be paid prior to the first game (late August for volleyball/Mid-November for basketball). For children playing both sports, the deposit for volleyball is carried forward for basketball.
Deposits must be made in the form of a $40 check post-dated 5-15-09 payable to SJA Athletic Association. Do not combine deposits for multiple children – a separate check is required for each athlete. Checks will be destroyed provided uniforms are returned in good condition.
Please send your post-dated deposit check(s) to the office in an envelope marked “Athletic Assn/Uniform Deposit” prior to the start of the season. Please indicate athlete’s name in the memo section of your check.
